In the U.S., despite extensive health knowledge, life expectancy is declining due to preventable diseases and health inequities. This troubling trend highlights the need for more than just knowledge—it demands equitable, efficient, and actionable public health strategies. PRISM (Population Research, Innovation, and SysteMs for health equity) is a pioneering initiative aimed at transforming population health. It will establish the first-ever learning population health consortium, integrating healthcare systems, wearable technology, patient-generated data, public health departments, and community networks into a unified, dynamic learning system. PRISM’s real-time approach will continuously gather evidence, implement interventions, and monitor health outcomes across diverse communities, allowing for immediate improvements and adjustments. This rapid learning and action cycle aims to foster measurable health improvements across the U.S., ushering in a new era of population health innovation.
Led by Northwestern University with joint partnership with the University of Florida, the Louisiana Public Health Institute, and Patient is Partner LLC, PRISM brings together health systems and communities nationwide, including institutions like Vanderbilt, Mayo Clinic, and Johns Hopkins. PRISM is committed to driving local and national changes to achieve health equity, restoring public confidence in effective health strategies and enhancing well-being for all.
