Dissemination and Implementation
As we gather information from both patients and providers, the STELLAR Center is committed to not only sharing our research findings but also providing lesson plans and modules for effective implementation. Our goal is to implement and integrate patient-reported outcomes assessments and employ EDW for smoking cessation, physical activity, and weight management programs throughout the entire Northwestern Medicine (NM) oncology care system, while optimizing resource allocation. The insights from this rigorous pragmatic study will be applicable to diverse clinical settings and patient populations. Moreover, they will enhance the evidence base for telehealth services, potentially transforming cancer care.
Outcomes Measurements
For PRIMARY OUTCOMES, we will evaluate the effects of the STELLAR Facilitated program compared to Self-Guided program on reach and access to potential and current STELLAR participants.
Reach and Access
- Number of candidates identified from the EDW
- Number of patients contacted and offered trial enrollment
- Number of participants randomized to the STELLAR study
- Number of participants who attend >1 coaching session
For SECONDARY OUTCOMES, we will evaluate the effects of the STELLAR Facilitated program compared to the Self-Guided program on health behaviors, healthcare cost and care utilization, health outcomes (symptoms and quality of life), and many more.
- Health Behaviors
- Smoking, Body Weight, Physical Activity
- Healthcare Costs and Care Utilization
- Unplanned, all-cause hospital admissions; potentially avoidable, all-cause emergency department use; 7-day hospital readmissions; and use of triage clinic
- Care Quality
- Patient-Provider Communication and Shared Decision-Making
- Symptoms and Functioning
- Quality of Life
