The goal of this form is to identify discrimination and work towards reducing it within the department.
With your permission we may share a de-identified description of the complaint (a) with other students in town halls or in our annual report ; (b) in an email to the the department chair (Rick Zinbarg); and / or (c) during a faculty meeting to highlight the kinds of behaviors that are harmful to students. Due to its anonymous nature, this information will not result in a formal complaint. If you would like to file a formal complaint at any point, you can email any of the DIC members or the current Director of Graduate Studies (Satoru Suzuki) to begin this process.
