Adjunctive Glucocorticoid Therapy in Patients with Septic Shock (ADRENAL Trial) – NEJM, 2018
- Landmark study of hydrocortisone 200mg daily by continuous infusion vs placebo in 3,658 mechanically ventilated patients with septic shock (defined as use of vasopressor for at least 4 hrs). No difference in 90-day mortality between groups, but patients in the intervention arm with faster shock resolution, shorter time to liberation from mechanical ventilation, and shorter time to ICU discharge. Importantly, there was no increase in bacteremia or fungemia in the steroid arm. Steroids were continued for 7 days or until ICU discharge.
