*Who Can Join All of Us at Northwestern?

• 18 years of age or older
• Currently live in the United States

6 Steps to Join (Initial Enrollment 75-90 minutes):

Steps 1-5 should be completed prior to the in-person appointment.

1. Create an account
2. Give your consent
3. Agree to share your Electronic Health Records (EHR)*
4. Complete the Consent to Get DNA Results
5. Answer health surveys
6. Complete your in-person appointment by having your measurements taken (height, weight, blood pressure, etc.) and give blood and urine samples, if needed**

After completing these steps, you’ll receive a one-time $25 gift card.***

*Sharing your EHR is your choice. If you decide not to share your EHR, you can still participate by answering health surveys. But you will not be asked to take part in some other aspects of the program.

**You can also participate from the comfort of your home by requesting an at-home saliva collection kit and submitting remote physical measurements through your participant portal. If you choose to participate from home, you may be invited to donate a blood sample in-person in the future where you can receive $25.

***Participants receive a $25 payment and complimentary parking (or, for the Chicago Downtown location only, a 24-hour Ventra Card) for their appointment.

Upon arrival for your in-person appointment, here’s what to expect:

• You will be asked to self-report any COVID-19 symptoms.
• In accordance with the Centers for Disease Control and Prevention and the Illinois Department of Public Health guidelines, we require universal masking.
• A mask should cover both your mouth and nose at all times throughout the facility, including our clinic rooms. If you do not have one, you will be given a Northwestern Medicine mask upon check in.
• Aside from essential caregivers, no visitors are permitted at this time. Please let a staff member know if you require additional accommodations. (Please note: all visitors must be 18 years and older.)
