Together

we can change the face of medicine

The future of health begins with you

The All of Us Research Program has a simple mission. We want to speed up health research breakthroughs. To do this, we’re asking one million or more people to help us lead the way in discovering better care for all of us both in the future and today.

We have resumed in-person appointments with new safety precautions in place. If you have questions about participating in the All of Us Research Program, please call us at 312-695-6077 and leave a voicemail. A staff member will call you back within one working day.

What is the All of Us Research Program?

Creating the right health approaches and care for the right person is called precision medicine. Getting the right information to make that happen is the goal of the All of Us Research Program from the National Institutes of Health (NIH).

To get there, we want to create the largest health database ever. By understanding people’s health, neighborhood, family, and lifestyle, researchers will have information to better understand health and disease. This information may help create a healthier future for generations to come.

What is Northwestern Medicine’s role in the All of Us Research Program?

Northwestern Medicine is a proud partner of the All of Us Research Program in the Greater Chicago region. Northwestern Medicine is also a leading member of the Illinois Precision Medicine Consortium – a group of healthcare providers and fellow partners of the All of Us Research Program that represent a large and diverse patient population covering the greater Chicago area and Illinois. All of Us is run by the National Institutes of Health.

Northwestern Medicine is calling on individuals like you. We hope you will help make history and change the future of health. The health data you share will be added to the All of Us Research Program database. Researchers can access this data to conduct studies.

This could help them see how different factors, like environment, lifestyle, and genes, can impact health. This may help speed up health research breakthroughs.

How Does It Work?

01

Create an Account

If you decide to join the All of Us Research Program, the first step is to enroll by filling out this from, then you will create an account. You can do this through our registration page or at one of our participation centers. You’ll need to provide your name and email address or mobile phone number to enroll.

03

Agree to Share Your Electronic Health Records (EHR)

As part of the consent process, we will ask you to agree to share your electronic health records (EHR). Sharing your EHR is your choice. If you decide not to share your EHR, you can still participate by answering health surveys. But you will not be asked to take part in some other aspects of the program.

05

Answer Health Surveys

You will be asked to answer several health surveys. These are available on the Participant Portal. You can answer the surveys at your convenience using your computer, tablet, or smartphone. Your name will never be on these. Your answers will help build the All of Us database. Approved researchers can access this database to conduct their studies.

07

Ongoing Engagement

We are just getting started. Over time, opportunities to take part will grow. You can be involved as much or as little as you’d like. We may ask you to complete more health surveys, share physical measurements, or provide additional consent for new research studies.

02

Review the Consent

Once you’ve created your account, you will be asked to give your consent. This includes reviewing a series of videos, which will provide more detailed information about the program. These describe all the types of information you may be asked to give. Once you feel comfortable that you know what to expect, and if you are ready to give your consent, you’ll electronically sign the consent form. You can do all of this from your computer, tablet, or smartphone, or at a partner center.

04

Complete the Consent to Get DNA Results

Your DNA results could include information about your ancestry (which is where your family comes from), your genetic traits, whether you have an increased risk for getting certain health conditions, and how your body might react to certain medications. You may want to know this information or you may not.

06

Visit a Partner Center

You may be invited to visit one of our partner centers to have your physical measurements taken (height, weight, blood pressure, etc.) and provide biosamples (blood and urine). You will be able to choose the partner location most convenient for you. You can schedule this visit at Northwestern online at YellowSchedule.

01

Register

Fill out this form and register at the national website joinallofus.org.

02

Enroll

Complete the online consent process, answer health surveys and safely share your health data. If you prefer, you may complete the process at our participant center. Our staff there can help you.

You will then be an official participant in the All of Us Research Program.

03

Participate

All participants will answer questions. These questions may be about lifestyle, environment, and health history. You will also be asked to share your electronic health records. We will schedule you at our centrally located enrollment center for a study visit. At this visit, you will be asked to have your physical measurements taken and give biosamples (blood and urine). This appointment is at no charge.

04

Contribute

We are creating the largest health data resource ever. Your health information will be added to the All of Us Research Program database. Researchers may use this data in health studies. This may lead to the next big medical breakthrough.

Ongoing Engagement

We are just getting started. Over time, opportunities to take part will grow. You can be involved as much or as little as you’d like. We may ask you to complete more health surveys, share physical measurements, or provide additional consent for new research studies.

You will always have access to the data you share. This may be interesting to you, and help you learn about your health. We will tell you about the research studies that use All of Us Research Program data.
